Why Pregnancy Loss After Infertility Is More Than One Loss

When you get a positive test after years of trying to get pregnant, the joy is usually accompanied by some fear. After so long, your efforts finally worked—and now there’s something that could be taken away.

If that pregnancy, in turn, ends in a loss, there’s a distinct type of grief along with the disappointment and sadness, as well as a new kind of fear that compounds on top of the initial fear. This compounded fear comes from knowing now how painful it is, both physically and emotionally, to lose a pregnancy. This loss is layered because you are now grieving more than one loss.

The Loss of 'It Finally Worked'

If you've been struggling with infertility, getting pregnant is a relief. It finally worked, and you know your body can actually get pregnant. Losing that pregnancy can feel like having no say over what happens, with no guarantee that next time will be different.

Beyond proof your body can get pregnant, it can feel like the hard part is over. It may be the first time in a long time that you've let yourself plan.

If you've been in treatment, there’s often a moment of graduating from the fertility clinic to regular pregnancy care. After a loss, you go back to the clinic, the waiting room and the protocols, and it can feel like starting over from zero.

You may have followed every instruction this time, and it still ended, which can make the loss of control feel even greater.

Grieving More Than One Loss

When we think of grief, it is basically a response to loss. A loss can be tangible, emotional, or even imagined.

A pregnancy loss after infertility is complex because it involves losses at all these levels.

There's the loss of that reproductive story we imagined. Clients usually describe this timeline loss as a disruption in the order of things, at least the order many of us expect.

Then we have losses along the way: the loss of control, identity, belonging, hope, innocence, and the loss of planning.

Infertility and pregnancy loss also involve grieving a future. What makes this grief different is that it’s for something that never happened, something you wanted but never had.

When pregnancy finally happens, especially after fertility treatments, we get to see the light at the end of the tunnel. Unfortunately, 1 in 4 pregnancies end in a loss, which can make that moment very brief (Koert et al., 2023).

The Loss of Innocence in Pregnancy

A loss can also take away the joy of celebrating when a pregnancy happens again. In session, this is often described as losing the innocence of getting pregnant before knowing that it can be lost.

After one pregnancy loss, the brain learns that loss is very much possible. From then on, it holds both possibilities: a pregnancy that could continue and one that ends in loss. Since this loss can be significant and traumatic, it is now focused on the negative possibilities mostly to keep you safe from future suffering.

When the next pregnancy happens, there's no more of that carefree joy of planning the nursery, choosing baby names, and picking out cribs and newborn onesies. You are now experiencing the mature side of pregnancy after loss, and with the maturity comes the bracing for negative results. Couples who became pregnant again after a loss have described this as never being “pregnant in the same way again” (Koert et al., 2023). Clinicians call this anticipatory grief, when part of you starts grieving a loss before it happens.

Pregnancy after loss brings its own fears, especially after infertility and fertility treatments. Part of what makes it so hard is trying to hold on to hope that this time could end well. I wrote more about this in an earlier post, “The Trauma of Repeated Hope in Infertility.”

The Loss of Time and Treatment

Having gone through fertility treatments brings on even more losses, such as loss of a cycle, the money spent, and sometimes the last embryo.

There’s also the time. Each cycle takes weeks of appointments and waiting, and after a loss there’s often another wait before you can try again, while you’re aware of every month that passes.

In a recent study, women going through fertility treatment described the process as an “extended present,” where time moves back and forth between hope and uncertainty (Karaca Biçakçı et al., 2026). They also talked about their bodies becoming the focus of medical attention, and about the pressure they felt from the people around them. The researchers described infertility as a “biographical disruption,” something that changes how we see ourselves and where we fit in with others.

This study looked at women in treatment, and I see the same thing happen after a loss. When a pregnancy ends after fertility treatment, it can pull you right back into that extended present, just when the end finally seemed in sight. And this time, hope can feel harder to reach.

Letting Go of the Struggle for Control

Once you’ve lost a pregnancy after infertility, loss is no longer something that only happens to other people.

It’s understandable to want control over what happens next, especially after everything you’ve done to get here. A lot of the pain comes from the struggle itself, from trying to make sure it never happens again when so much of it was never in your hands.

In therapy, a big part of the work after a loss is letting go of that struggle to be in control. Of course, this is hard to do when so much of trying to conceive has already been about losing control.

When the struggle starts to ease, it often becomes a little easier to hope again, even with the fear still there.
